Semaglutide vs Tirzepatide: A Simple Guide

Updated October 2026

TrimRx offers two GLP-1 medicines: semaglutide and tirzepatide. Both are weekly injections that help you feel full sooner and eat less. Here is how they differ, in plain words.

How they work

  • Semaglutide acts like a natural gut hormone called GLP-1. It slows down how fast your stomach empties and tells your brain you are full. It is the medicine in the brand-name drugs Wegovy and Ozempic.
  • Tirzepatide acts like two gut hormones: GLP-1 and GIP. It is the medicine in the brand-name drugs Zepbound and Mounjaro.

What the big studies found

Large studies of the brand-name medicines showed strong results when combined with diet and exercise:

  • In the STEP 1 study, people taking semaglutide 2.4 mg lost about 15% of their body weight on average over 68 weeks.
  • In the SURMOUNT-1 study, people on the highest dose of tirzepatide lost about 21% on average over 72 weeks.
  • In SURMOUNT-5, which compared them directly, tirzepatide led to about 20% weight loss and semaglutide about 14% over 72 weeks.

These studies used the brand-name medicines. TrimRx provides compounded versions, made by a state-licensed pharmacy for each patient. Compounded medicines are not FDA-approved products, so your provider guides your dose and checks how you are doing.

Side effects

Both medicines have similar side effects. The most common, according to the Wegovy and Zepbound labels, are:

  • Nausea
  • Diarrhea
  • Vomiting
  • Constipation
  • Stomach pain and indigestion
  • Tiredness

These often ease as your body gets used to the medicine. Starting at a low dose and going up slowly helps. Tell your provider if side effects bother you.

Who should not use them

Do not use either medicine if you or a family member has had medullary thyroid cancer or a condition called MEN 2, if you have had a serious allergy to the medicine, or if you are pregnant or planning a pregnancy.

Which one is right for me?

Your TrimRx provider will help you decide, based on your health, your goals and your budget. Semaglutide costs less per month. Tirzepatide acts on two hormones and showed larger average weight loss in studies of the brand-name medicines.
